We Work Alongside You

Harbors North is a specialty clinic focused exclusively on advanced, FDA-approved treatments for treatment-resistant depression — Spravato (esketamine) and Transcranial Magnetic Stimulation (TMS).

We accept referrals from PCPs, therapists, psychiatrists, and psychiatric nurse practitioners throughout Northern Michigan. Our role is to provide targeted interventions that complement your existing care plan — not to replace the primary care relationship you've built with your patient.

When you refer a patient to us, we screen the referral, conduct an initial assessment, and if they're a good candidate, begin treatment. You stay in the loop with progress updates, and your patient continues their ongoing care with you.

Who Qualifies for Treatment

Below are the general qualifying criteria for each treatment. If you're unsure whether a patient is a good fit, we're happy to discuss — just call our office.

Spravato (Esketamine)

Spravato Criteria

Spravato is FDA-approved for treatment-resistant depression (TRD) in adults. General criteria include:

  • Diagnosis of major depressive disorder (MDD) — not bipolar depression (Spravato is not indicated for bipolar depression and payers will not reimburse)
  • Inadequate response to at least two adequate trials of antidepressants in the current episode
  • No active psychosis
  • No uncontrolled hypertension or aneurysmal vascular disease
  • Note: some payers require a concomitant oral antidepressant — requirements vary by plan

Common ICD-10 Codes

F33.1, F33.2 (Recurrent MDD, moderate/severe)
F33.9 (Recurrent MDD, unspecified)

TMS Therapy

TMS Criteria

TMS is FDA-approved for treatment-resistant depression in adults. General criteria include:

  • Diagnosis of major depressive disorder (MDD)
  • Inadequate response to at least one adequate trial of antidepressant medication
  • No metallic implants in or near the head (cochlear implants, aneurysm clips, etc.)
  • No history of seizures or epilepsy

Common ICD-10 Codes

F33.0, F33.1, F33.2 (Recurrent MDD)
F32.1, F32.2 (Single episode MDD, moderate/severe)

What Makes a Strong Referral

The more information you include, the faster we can screen and schedule your patient.

A Strong Referral Includes

  • Pharmacy fill list going back 3–5 years (to document prior antidepressant trials)
  • Current medication list with dosages
  • Current diagnosis and relevant psychiatric history
  • Recent PHQ-9 or MADRS scores
  • Any relevant medical history (especially cardiovascular for Spravato)
  • Patient's insurance information

May Not Be the Right Fit

  • Patient has not yet tried any antidepressant medications
  • Primary diagnosis is not depression (e.g., personality disorder, primary substance use disorder)
  • Patient is seeking general psychiatry or therapy services
  • Uncontrolled hypertension or history of aneurysmal vascular disease (for Spravato)
  • Metallic implants in or near the head, or history of seizures (for TMS)
  • Existing or history of psychosis
  • Bipolar depression (Spravato is not indicated and insurance will not cover)
