How It Works
Submit your referral below. You'll receive immediate confirmation on screen and by email (if provided).
Our team will review and follow up by fax within 2–4 business days. If we need additional records, we'll let you know exactly what's needed. If your patient is a good candidate for treatment, we'll begin coordinating intake.
Your patient stays under your care. We collaborate with you throughout treatment and provide regular progress updates by fax.
