What this covers

This policy covers harborsnorth.com, the website. It is not our Notice of Privacy Practices, which explains how we handle your medical records as a patient. You receive that notice when you become a patient, and you can ask for a copy any time by calling (231) 829-7126.

What you give us

Contact form. Name, email, phone, what you're asking about, an optional few questions that help us prepare for the call (which treatment, how long you've struggled with depression, who prescribes your medications, your insurance carrier), and your message. The form runs on JotForm under a HIPAA business associate agreement, and submissions go only to our office team.

Referral form. Used by healthcare providers to refer a patient. It collects the provider's details and the patient's name, date of birth, insurance, diagnosis, treatment history, and any records the provider attaches. It also runs on JotForm under a HIPAA business associate agreement. Our team reviews every submission.

Phone and email. If you call or email us, we keep what you tell us so we can follow up. Ordinary email is not encrypted; if you'd rather not send health details by email, call us instead.

What our website collects on its own

Like most websites, we use analytics to understand how people find and use the site. We use Google Analytics, Google Ads conversion tracking, and the Meta (Facebook) Pixel. These tools use cookies and similar technology to record things like which pages you visit, how you arrived, whether you tapped the phone number or submitted a form, and general device and location information. They do not read what you type into our forms.

You can limit this by using your browser's private mode, blocking cookies, or using an ad-blocker. Google and Meta each publish their own privacy policies and opt-out tools.

How we use it

To respond to you, to screen referrals, to understand which pages and ads bring people to us, and to improve the site. That's it.

What we don't do

We do not sell your information. We do not share form submissions with anyone outside Harbors North and the services named above that process them for us. We do not use what you submit through a form for advertising.

How long we keep it

Form submissions are kept as long as we need them to respond and, for referrals, as part of the patient's record. Analytics data is kept according to Google's and Meta's retention settings.
